If you are interested in joining Alta Lodge, please fill out and submit the following form, which will be automatically emailed to the Lodge Secretary.  This information will not be shared outside the Lodge.  IT IS NOT A FORMAL APPLICATION, WHICH REQUIRES A HAND-WRITTEN “PETITION FOR MEMBERSHIP” FORM.  Comments by the fields will outline some of the requirements.

If you are not a male at least 18 years of age, have not resided in Illinois for the last six months, have a criminal background, or do not believe in a Supreme Being, STOP HERE — You are NOT eligible for membership in the Masonic fraternity.
